Transforming End-of-Life Care with Humber and North Yorkshire ICB

Black Pear is proud to collaborate with Humber and North Yorkshire (HNY) ICB in revolutionising end-of-life (EoL) care.

Together, we are aligning healthcare actions with patient wishes while delivering significant financial and environmental benefits.

Key Outcomes:

  1. 1,119 Ambulance Trips Saved Annually
    Access to EoL patient data enables paramedics to avoid unnecessary hospital conveyances, improving care outcomes and efficiency.
  2. Enhanced Patient Experience and Reduced In-Hospital Deaths
    Real-time data-driven decisions empower more patients to spend their final days in their preferred setting. Northern Lincolnshire leads by example, with only 14% of End-of-Life (EoL) patients passing away in hospitals, compared to the national average of 42–45%. This approach could potentially prevent 3,618 in-hospital deaths annually.
  3. Cost and Carbon Footprint Reduction
    Avoided hospital admissions and ambulance conveyances deliver financial savings and reduce the healthcare sector’s environmental impact.
  4. Validated Benefits
    Supported by the Yorkshire Ambulance Service (YAS), Northern Lincolnshire End-of-Life teams, and clinical experts, this program demonstrates tangible results in delivering patient-centered care.

This project highlights how technology and seamless data integration, ensuring the delivery of accurate information to the right place, can significantly enhance patient outcomes and boost operational efficiency.
